Formulary Lookup
The medications listed on the following formulary have been reviewed by the MedImpact Pharmacy and Therapeutics (P&T) committee, and have been found to be appropriate for formulary inclusion on the basis of safety, efficacy, approved indications, ease of use, potential for adverse effects, and cost effectiveness.
The MedCare Formulary is the MedImpact national formulary, and it is updated quarterly.

Submit a Prior Authorization Request
The Medication Request Form is submitted by participating physicians and providers to obtain coverage for a formulary drug requiring prior authorization (PA), a non-formulary drug for which there is no suitable alternative available, or any overrides of pharmacy management procedures such as step therapy, quantity limit or other edits.
